One South West (OSW) Shared Care Records Programme, a group of six Sustainability & Transformation Partnerships (now ICBs), came together to collaborate on a solution to allow the sharing of information to improve the direct care of patients across health and social care. Digital maturity varied across the region, with several existing Shared Care Record solutions in place, and the case for integrating across the area included:

• Improving the quality of care, patient safety and experience.
• Delivering greater time efficiencies for staff.
• Reducing admissions to hospital.
• Lowering the cost of care.

IQVIA were selected for the project, and following a period of consultation and working across the ICBs and various care organisations, the team outlined a clear strategy of action for an effective and efficient way of sharing citizen information between authorised health and care professionals across the South-West and beyond. Stages included:

• Finding a direct care solution for Devon & Cornwall, securing funding, conducting procurement, and commencing implementation.
• Working with stakeholder organisations to develop a strategy for connecting existing solutions across the region.
• Supporting stakeholders with planning around Population Health Management.

Single source is saving time: Pre-op assessment prep used to involve lots of paperwork and take 4 hours, now done in minutes. Plus, less GP time spent on admin and reviewing future appts.

Improved data access: Streamlining the process of carrying out child protection reviews

Optimising efficiencies: GPs are now able to access hospital-based clinical assessments, avoiding the need for referrals.

Supporting development: The team helped to create a Full Business Case for the Devon & Cornwall direct care solution.

• Patients diagnosed with solid tumours, for example breast, lung and prostate cancer, often face complications related to their bones.
• Longer waiting times for specialised surgical treatments, along with the effects of severe pain and reduced mobility, have a negative influence on quality of life.
• There are higher costs associated with managing cancer patients with bone metastases, along with higher admissions and increased hospital stays.

• To enhance patient outcomes and optimise NHS resources, IQVIA analysed treatment delivery in secondary care using Cost Benchmarking data via IQVIA HOB.
• This research project utilised aggregated data, including pseudonymised information on inpatient care, outpatient consultations, emergency visits, medicines dispensed, and pathology tests.
• Through this data we demonstrated that effective treatment reduces fractures and bone issues, improving quality of life, and decreasing healthcare resource use - thus easing the burden on NHS services.

• Improved Patient Outcomes: Proactive management of patient complications leads to improved outcomes and quality of life
• Reduction in patient fractures: Patients on the treatment had fewer visits to A&E and fewer admissions
• Reduced length of stay: Reducing the duration of treatment also reduced costs and resources, easing burden on services
