Frequently asked questions about Japan medical device registration process & Designated Marketing Authorization Holder (DMAH)

Medical devices in Japan must follow Pharmaceuticals and Medical Device Agency (PMDA) approval process under the PMD Act, including approval (Shonin), certification by notified body, or notification depending on classification of the device. Approval is required for higher-risk and novel devices.
To market a medical device in Japan, manufacturers must obtain regulatory approval and operate through a local Marketing Authorization Holder (MAH) or Designated Marketing Authorization Holder (DMAH).
The MAH is the domestic entity that holds the marketing approval, certification, or notification for the product. In contrast, the DMAH is the domestic entity appointed by the foreign manufacturer (Foreign Special Approval Holder) to carry out domestic responsibilities in Japan on its behalf.
Yes, however, a planned and well-controlled transition is required. When changing the DMAH, activities such as the transfer of regulatory documentation, handover of QMS and GVP systems, transfer of product technical documentation and labeling (including package inserts), as well as notifications and filings with the regulatory authorities will be necessary.
Yes, companies can outsource specific functions such as regulatory file development based on their needs. Please contact us for further details.
PMDA consultations provide regulatory guidance on clinical development, data requirements, and submission strategies, helping align development with regulatory expectations.
Clinical data requirements depend on device novelty and equivalence. Some devices can leverage existing data from foreign studies, while others require local clinical trials. We can help to develop the strategy to meet your goal.
Reimbursement in Japan is determined under the National Health Insurance (NHI) system, where the government establishes pricing and coverage.
The reimbursement pathway depends on whether the device can be covered under an existing category or requires a new evaluation. Certain new medical devices may require MHLW and Chuikyo review for reimbursement and pricing.
Yes, definitely. Regulatory interactions with PMDA and MHLW are conducted in Japanese and require local expertise and experience.
