Cette technologie pourrait à la fois permettre d’optimiser les performances du système de santé et de mieux calibrer l’allocation des ressources. Soigner mieux de façon plus « cost effective », tel sera l’enjeu, notamment pour dégager des marges de manœuvre susceptibles de financer l’innovation thérapeutique. Oui, mais… « Cette révolution nécessitera des investissements technologiques, des compétences spécifiques et des ajustements réglementaires », prévient d’emblée Michèle Arnoe, directrice de l’innovation chez IQVIA France.
Améliorer le suivi des parcours de soins
Pour se démarquer de la simple analyse statistique, il faudra affiner la donnée brute, mais aussi l’enrichir, en croisant différentes sources. « Si l’on veut appréhender tout le continuum de la prise en charge, la donnée longitudinale devra être analytiquement comparable pour déterminer les causes et les effets des différents événements sanitaires », souligne Michèle Arnoe. L’intelligence artificielle présente un double intérêt en la matière. Elle permettra non seulement de structurer la donnée, mais également de nourrir des hypothèses fiables, à partir d’une donnée enrichie. « La finesse et la qualité des données collectées en vie réelle détermineront en grande partie la pertinence des résultats proposés par la machine. Quand nous aurons atteint ce niveau d’excellence, nous pourrons développer des algorithmes élaborés qui contribueront à améliorer significativement le suivi des patients au sein des parcours de soins. L’approche thérapeutique sera davantage personnalisée dans le cadre d’une médecine de précision », assure-t-elle. L’analyse prédictive semble donc promise à un bel avenir, mais son essor reste suspendu à l’évolution de la réglementation actuelle.
Un équilibre à trouver
L’ouverture des bases de données publiques est salutaire, mais la manière dont elle est régulée pourrait effectivement s’avérer contre-productive. Entre l’indispensable protection des libertés individuelles et la perspective de brider l’innovation, la frontière est extrêmement mince. « Il y aura un équilibre subtil à trouver, confirme Michèle Arnoe. Le circuit d’autorisation gagnerait à être plus souple, plus flexible et plus homogène au niveau européen. Dans l’intérêt général, nous pourrions parfois bénéficier de données non brouillées, en particulier pour des problématiques pointues liées à la recherche ou à un accès précoce aux soins. En contrepartie, nous nous devrons d’être des experts neutres et des partenaires de confiance. » L’idée d’un système à géométrie variable selon les besoins et la finalité de l’étude paraît cohérente, mais elle est incompatible avec la tradition jacobine française. D’autres pays moins timorés, à l’instar de la Chine, pourraient largement profiter de l’aubaine.
